Jamie J. Cavanaugh is currently the executive director of population management at UNC Health. In this role, she oversees a wide array of care management services across community primary care sites and transitional case management for patients discharged from over 13 hospitals and emergency departments throughout North Carolina. She leads a multidisciplinary team composed of nurses, dietitians, and social workers, while working closely with the department of pharmacy. Cavanaugh has successfully expanded chronic care management and collaborative care management services across community primary care sites. She provides leadership for the Continuing Care Network, which includes 22 affiliated skilled nursing facilities and eight affiliated home health agencies, as well as the complex case management program.

Cavanaugh earned her Doctor of Pharmacy from the University of North Carolina at Chapel Hill, Eshelman School of Pharmacy. After graduation, she completed a PGY1 residency at Durham VA Medical Center and a PGY2 ambulatory care specialty residency at UNC Health. With 11 years of experience as a clinical pharmacist practitioner in an academic internal medicine clinic, Cavanaugh has made significant contributions, including establishing an ASHP Best Practice Award-winning transitions of care service. She served as a pharmacy student and resident preceptor from 2012 to 2021, offering 180 learner rotations during that tenure, and received the UNC Eshelman School of Pharmacy Preceptor of Excellence award in 2019. She has also served as an operational leader for care managers, advanced practice providers, and pharmacists.

Cavanaugh has made sustained contributions to the ASHP Section of Ambulatory Care Practitioners, serving as director-at-large for the executive committee and chair of the Compensation and Practice Sustainability Advisory Group. She has presented at numerous ASHP meetings and contributed to various member resource documents. Her work has been published in a variety of peer-reviewed journals, showcasing her dedication and impact in the field of pharmacy and healthcare management.
