Todd D. Lemke

2025 Distinguished Service Award Winner

Todd D. Lemke is the director of regional pharmacy services for CentraCare located in central Minnesota. Lemke joined Paynesville Area Health Care System after completing a PGY1 residency and started a rural hospital inpatient practice along with being part of the ambulatory pharmacy program that worked alongside other providers caring for diabetes and other chronic health conditions. When the rural health system became part of CentraCare, Lemke became active on the system level for immunization decisions and improving chronic disease management in the clinics. He helped expand and lead the system’s medication therapy management program and integrate and standardize the health system’s anticoagulation services departments into one comprehensive program. Lemke currently oversees three of CentraCare’s hospital pharmacy departments including a community hospital, a cancer center and two critical access hospitals. He has overseen the education of many pharmacy students and residents over the years and continues to be involved in the PGY1 residency program at his site. 

Lemke graduated with his Doctor of Pharmacy from the University of Minnesota College of Pharmacy in 1999. His pharmacy residency was used to complete initial ASHP accreditation for the University of Minnesota ambulatory care residency program in 2000. Lemke maintained a Certification in Diabetes Education for 20 years in clinical practice.

Lemke is an active member and recognized Fellow of ASHP. He completed the ASHP Pharmacy Leadership Academy in 2023. Lemke is currently chair for the ASHP Council on Pharmacy Practice, has served as chair for the ASHP Section of Inpatient Care Practitioners Executive Committee, the section’s Committee on Nominations and Advisory Group on Small and Rural Hospitals. He has served on a joint ASHP/IMSP insulin safety advisory group. He is also involved with both of Minnesota’s pharmacy organizations, MSHP, and MPhA, the Minnesota Rural Health Association and the National Rural Health Association. He has served as president of the board for the Minnesota Rural Association and was a five-year faculty member for the HRSA Collaborative on Patient Safety and Clinical Pharmacy Services. Lemke has talked at numerous pharmacy and health care conferences and college of pharmacy classes over the years.
