This webinar is a continuation of ASHP’s support of evaluating and identifying opportunities for health system pharmacists and pharmacy to engage in the emerging hospital at home care models. Our speaker(s) will describe models where pharmacy clinical services are being incorporated into hospital at home care as well as the organizational structure necessary to manage and provide oversight for the medications being provided to the patients in their home. Interaction and audience participation is encouraged.

After completion of the live webinar, you will process and claim CPE credit online at elearning.ashp.org.
