Zach Weber

Zachs Story

Dr. Weber received his Pharm.D. degree from Purdue University and completed a PGY1 Pharmacy Practice residency at the Roudebush VA Medical Center in Indianapolis, Indiana followed by a PGY2 Ambulatory Care Specialty residency at Duke University Hospital in Durham, North Carolina. He obtained BCPS certification in 2008, CDE certification in 2011, and BCACP certification in 2012.

Dr. Weber is the current Chair for the Compensation and Practice Sustainability Section Advisory Group.

Facility

Dr. Weber currently works as a faculty member with Purdue College of Pharmacy and Ambulatory Care Clinical Pharmacy Specialist with Eskenazi Health. Eskenazi Health is a tertiary care, academic medical center in Indianapolis, Indiana with over 15,000 adult inpatient admissions and nearly 100,000 outpatient visits annually.

Recent Significant Projects

Significant projects for Dr. Weber include the expansion of collaborative drug therapy management services at Eskenazi Health from 1 pharmacist at 1 clinic, to 5 pharmacists at 7 clinics. He has also helped in the development of a new pharmacist-managed transitions of care clinic. He has also served as a research advisor for the development and assessment of a pharmacist-managed rivaroxaban clinic.

Becoming Involved in ASHP

Dr. Weber initially became involved with ASHP, "to meet other people who have been successful in the expansion and sustainability of pharmacy services. To learn about best practice models, and gain access to resources that can help in the expansion or improvement of pharmacy services at my institution. Since we have also been successful with the development and expansion of ambulatory care clinical pharmacy services at my practice site, I now really enjoy the opportunity to share our successes (and things learned) with others who are looking to do the same thing.

Advice for Someone New to Specialty Area

“The most important initial step to the expansion and sustainability of ambulatory care pharmacy services is to identify key stakeholders that will allow you to practice in new areas where pharmacists might not have been before (or to expand services in existing areas with pharmacy services).”

Value of ASHP

“The biggest value of ASHP to me is related to access. I really enjoy how an organization like ASHP can make the seemingly overwhelming world of pharmacy practice just that much smaller. It provides me with access to other pharmacists and practice models that I can use as examples. It provides me with access to educational materials and documents that I can use as resources. And, it provides me with access to opportunities to connect with my peers, and to give back to the profession through positions like my current one as Chair of the Compensation and Practice Sustainability Section Advisory Group.”
