Ongoing Certification Requirements and List of ASHP-Certified Organizations
Annual reports are required for every certified pharmacy service and reviewed by the Pharmacy Practice Accreditation Commission (PPAC) and Practice Advancement Office; more frequent reports may be requested in the judgement of the Commission.
Recertification
Recertification occurs every three years, repeating the certification process as described.
