Will HammondsWill Hammonds, PharmD, MS, BCPS is a pharmacy manager overseeing ambulatory care and transitions of care services at WVU Medicine - WVU Hospitals in Morgantown, West Virginia. Will played a pivotal role in implementing the antithrombotic stewardship program and enhancing roles for pharmacists and technicians in direct patient care and care transitions. By integrating innovative strategies and fostering collaboration, Will strives to elevate the quality of care and support seamless transitions between healthcare settings.

Patricia ValdezPatricia Valdez, CPhT is a clinical pharmacy technician at WVU Medicine’s J.W. Ruby Memorial Hospital and founding member of the Antithrombotic Stewardship Program. She became a CPhT in 2002 and has since gained extensive experience across the independent and retail settings, as well as the inpatient hospital environment. Patricia is passionate in assisting patients with medication needs and is dedicated to collaborating with the healthcare team to ensure all patients can obtain medication without financial hardship.
