Primary Care Transformation Ad Hoc Committee
The purpose of the Ad Hoc Section of Ambulatory Care Practitioners (SACP) Primary Care Transformation Committee is to identify key strategic priorities for advancing primary care transformation within health system pharmacy practices, while striving for identification of the ideal primary care model as proposed by the 2024 ASHP Commission on Goals (COG). The Committee will assess initiatives and resources required by health system pharmacy practitioners and leaders as well as non-pharmacy leaders in these settings and take the lead in developing or delegating resource development across the SACP Executive Committee (EC) and Section Advisory Groups (SAGs) to address those needs.
